About this opportunity

The Health Equity Resource Communities (HERC) program provides grant funding and state resources to local community organizations to address health disparities, improve health outcomes, expand access to primary care and prevention services, and help reduce health care costs. Established under the Maryland Health Equity Resource Act (2021) and implemented by the Maryland Community Health Resources Commission (CHRC), the program awards 5-year grants to organizations delivering clinical interventions and social determinants of health (SDOH) support for marginalized populations across Maryland. Twelve grantees are currently implementing programs over the period July 1, 2024 through June 30, 2029, with a total funding pool of $41,550,000 across 12 awards.
