About this opportunity
This program funds community-based organizations to provide interventions that address social determinants of health (SDOH) — upstream, non-medical factors outside of health care such as housing, transportation, education, income, food access, racism, and discrimination that affect health outcomes — in order to reduce health inequity among Maryland's racial and ethnic minority communities. The goal is to reduce health disparities by supporting community-based interventions that target these upstream factors. Grantees are monitored through narrative reports and performance measures during the funding period.
